Many on the right are seething over the fact that homosexuality is no longer considered an illness, and transsexuality has been relegated from mental health diagnoses to sexual ones.
But we've begun to forget that sadism could have been a real diagnosis, but for some reason it was removed.
According to the DSM-III-R, the diagnostic criteria were defined by a pervasive pattern of sadistic and cruel behavior that began in early adulthood. It was defined by four of the following (checkboxes are placed where they match the behavior of the mainstream right):

• Has used physical cruelty or violence for the purpose of establishing dominance in a relationship (not merely to achieve some noninterpersonal goal, such as striking someone in order to rob him/her).
• Humiliates or demeans people in the presence of others. ✅ Doxxing people online, humiliating liberal students during debates.
• Has treated or disciplined someone under his/her control unusually harshly. ✅ ICE
• Is amused by, or takes pleasure in, the psychological or physical suffering of others (including animals). ✅ I don't think I need to remind you how chuds treat trans people and what kind of rightoid "memes" there have been in recent years.
• Has lied for the purpose of harming or inflicting pain on others (not merely to achieve some other goal). ✅ Propaganda that almost half of trans people commit suicide.
• Gets other people to do what he/she wants by frightening them (through intimidation or even terror). ✅ For example, intimidating gender dysphoric teens so that they transition late and become another target for humiliation.
• Restricts the autonomy of people with whom he or she has a close relationship, e.g., will not let spouse leave the house unaccompanied or permit teenage daughter to attend social functions. ✅ HRT gatekeeping
• Is fascinated by violence, weapons, injury, or torture. ✅ Nazi aesthetics

I imagine bourgoises in important positions got diagnosed with it, potentially disqualifying them from stuff, so they pressured the DSM to remove it.

It's much easier and simpler to be a sadist than to be a more normal human being who relates to other people and negotiates nuanced interactions. Relying solely on your basal lizard brain of "kill and eat everything for my enjoyment" is the route of laziness and stupidity. For some of these people it's a consequence of incapacity (likely from brain damage due to overabundance of lead and other environmental contaminants), but for most I would wager it's primarily laziness and/or addiction to the path of least resistance. Basically, if you allow people to get away with this kind of behavior, you will encourage the laziest among us to do that because they simply don't have any incentive to do anything more difficult like develop self control or try to see things from another person's perspective.

Paradoxically, the more you alienate people from violence and their own physical abilities (i.e. having the state monopolize violence and use of force to mediate societal conflicts), the more you enable violent tendencies among the laziest and most aggressive people. This is the case both among those who join law enforcement (as is usually discussed on this topic) but also with those regular civilians who share those violent tendencies. Not only will the "civilized" men of the state have solidarity with the average chud, they will selectively use the state apparatus to repress only those who fight back or defend themselves. In a more "barbarous" society, where less of the physical power is held exclusively by the state, no such imbalance exists and everyone (most of whom are not the chuds but normal human beings) is more able to dis-incentivize that sort of destructive antisocial behavior.
