From your description of yourself you sound like a lot of people I know who range from passing to looking like an extremely fem twink (which is not the end of the world). 27 is a fine age tbh, I mean contrapoints was later and she looks good. Do you have a good support group IRL? When you have pretty people telling you you're pretty I think the brainworms will go away on their own.
Brow ridge and big nose are not really killer problems for the face, tbh nose is really overrated in uglyness factor and I wouldn't be ashamed of it. A bigger nose can actually help with other parts of the face because it distracts from them whilst not being a really masculine feature. Huge shoulders can be dealt with by clothing, or tbh you can just live with them. I have broad shoulders and I wear shoulderless / sleeveless clothing fine. I would experiment with sleeveless tops and see if it works on you. Adams apple does suck, it's kind of like bulge, you just have to live with it if you can't get surgery unfortunately. But that is one part of your body.
In general when it comes to transitioning, you can't worry about every part of your body. Passing (and I don't think passing is the be all and end all, looking good or sexy probably matters more. You can pass by looking like a grandma after all, but I'd rather be attractive and not pass perfectly) is more a sum of many things rather than one thing you do really well. So if you have a feature about yourself that you hate then it's probably fine if you can pull everything else off. I know you've got this!
I don't know if its an issue for you, but if you ever worry about falling into the 'ugly old trans' stereotype, then don't. The people you will see who actually do look like this are not doing so because they are coping, they look good in their own beauty standards. The kind of drag, extreme makeup and no effort into 'passing' is representitive of a completely different class culture of transfems coming from the lumpen-proletariat, whilst the imageboard /tttt/ culture you are coming from (idk if you came from there but it is a wider thing than just that website) values very different things and was born from a largely high paying worker / petit-bourgeois class background. You're not going to look like them no matter what, because you're not socialized to want to look like that.
It is important to point out that the flaws of the /tttt/ culture do in fact come from its petit-bourgeois background. That's why there is so much hyperfocusing on specific points of cranial features, and basically the bullying. It's vapid and representitive of a bourgeois view of beauty, which is only achievable through expensive surgery and which 99% of even cis people will never achieve.
