I'm in my mid 30s and this was literally me for over a decade with undiagnosed, unmedicated, untreated ADHD. What finally made me settle was running out of options. I had a kid, got a job, and suddenly I no longer had a choice. A lot of my avenues for life were shut down to the point that they were no longer even worth fantasizing about. Now that is not the path you should necessarily follow. But I point this out because I wanted to do all those things and in fact I did all those things badly because I never forced myself to specialize. I was a jack of all trades and not particularly good at anything. The job and the family really narrowed down my prospects so I'm forced to focus on just a few things. I choose exercise and diet. I gave up on writing novels after writing about 3 unfinished novels, one of which clocked in at over 60,000 words. I shared it with my friends and they never finished reading it because they were too busy or they secretly did and thought it sucked. Either way it wasn't encouraging. Then AI slop showed up and I realized it's worthless to write anything because even if you spend countless hours in a chair developing bad back, poor eyesight, and tailbone cysts, just to finish your magnum opus, other people will be able to pump out stuff of equal or higher quality with way less effort, and then your stuff will get accused of being AI anyway because most people aren't able to tell the difference even though they're super confident that they can. Basically if someone doesn't like your writing, they'll call it AI slop even if it isn't, and if they like it, they'll assume it's real even if it's AI slop. I have given up on art and have chosen to focus on family, job, and health. Sounds conservative but I still have my Communist political views, not that I have much time to act on them.

Nicotine, ideally not cigarettes. But vaping or other forms don't have much downsides other than the cost and potential addiction. Nicotine is dopaminergic just like amphetamines so it's a true stimulant unlike coffee which only removes drowsiness which gives the illusion of stimulation.

Idk about alternative medication, but what works for ADHD is removing the distractions, or removing yourself from distractions. I cant do anything productive at home, but can spend hours working at the library because there is nothing else to do.

Also you can train your focus, consider it like a muscle, anything that requires your active attention will do, so something as simple as video games can do so and the best thing is that it is transferable but not everything will be as pleasurable, but your stamina will persist regardless of the activity.

Fuck that shit, don't listen to these assholes op. That shit is terrible for you, huge chunks of my childhood on em and I always felt so amazing when I stopped taking the shit. There's tons of other methods that don't involve getting hooked on uppers. Look into your diet, protein in the morning for example is a necessity. Exercise can help calm your mind. And honestly, pick up multiple hobbies. At least cheap ones and maybe one costly one if you can afford it. I didn't figure out that music was my thing until I learned that I couldn't program, couldn't sculpt and preferred sound over drawing.
It's a pain in the ass journey, but adhd meds are just straight up bad for you.

Blogpost, but bear with me. I want mechanical vape, but vapes on the market are not-like-i-want-them. So I am learning how to CAD to make a vape, learning how mill and lathe work (not because I am planning on using them, but to orient myself on what is possible), what threads, tolerances, etc.
Basicially, to engage with a hobby you have to do concrete things. If you dont get it, read "On practice" by Mao again. You can only understand theory from concrete practice. You dont say

Forgot to add but autistic hyperfocus is something that you might have, for example while watching animays. You just have to channel it into different venues. If combined with ADHD, then you get extra fast context switching which will be extra helpful once you start yak shaving the task.
On learning stages yak shaving is good actually, it only becomes a obstacle once you get good at something. But by then autistic hyperfocus makes you not yak shave as much.

That's not a problem, you don't need to do a big leap every day, you can be content of doing something in the first place (and I know it's hard, but you SHOULD be happy about it, else you're gonna kill your motivation again)

I don't have advice for the ADHD stuff but I do have advice for the 'multiple things you want to do' thing:

In this case a 3D model would the destination, and the 34 hours spent 3D modelling would be the journey. Is it worth it? Only if you enjoy 3D modelling. Apply this to any and all desires you get. Eventually you'll discover something you enjoy doing.
