Self perception is a form of poison to the soul. We quite literally dont see ourselves the way we see others. I never notice people being fat or balding or having messy hair or having minor skin issues but can easily obsess over them in myself because I am looking for them in myself.

i don't know how it is for other people, but when i look at someone i don't really look at their face, i don't look at their superficial visual features or even really notice them, i look into their eyes, i look right past their face and see into their soul. i don't care about someone's outward presentation that is probably highly curated and largely disingenuous, i am interested in the genuine article underneath the mask, what is going on in their head. and i doubt that i am unusual in this regard, i imagine that a lot of people probably look at people this way, maybe even most people do it, even if they are not consciously aware of it. the reason i say this is, there is a good possibility that your obsessive hyperfocus on your superficial facial features is completely for naught with a lot of the people who see you or interact with you, they aren't even really looking at your face, they are seeing straight through your outer facade like it was as transparent as window glass and you are completely raw and naked and vulnerable, exposing all of your inner blemishes and insecurities all the time.
